Conduct comprehensive social assessments of patients’ social, economic, and environmental conditions and provide appropriate recommendations and social services.2. Prepare, maintain, and update patients’ social records in accordance with approved policies and procedures.3. Collect and document patient information, including family and social circumstances, to support the development of individualized treatment and care plans.4. Provide the healthcare team with relevant social information about patients when required.5. Conduct regular follow-ups with patients and maintain ongoing communication based on the nature of each case.6. Carry out home visits when required and in line with applicable regulations.7. Assist patients facing social or financial challenges by facilitating access to necessary medical supplies and coordinating referrals to social or charitable organizations.8. Prepare and maintain periodic statistical reports on social services provided to patients.9. Plan, organize, and coordinate social, cultural, religious, and awareness activities for patients.10. Organize and manage the loaning of selected medical equipment to patients in accordance with established procedures.11. Organize and deliver educational and awareness programs for patients and their families to promote health and social well-being.12. Support the organization and implementation of international medical and social awareness events.13. Attend meetings, workshops, and seminars related to social services.14.
