• Perform venipuncture and capillary blood collection in patients’ homes in accordance with approved policies, procedures, and infection prevention standards. • Verify patient identity using two identifiers (e.g., full name and MRN) before any specimen collection. • Review lab requests/orders (paper or electronic) for completeness, appropriate tests, and clinical information before collection. • Prepare all necessary phlebotomy equipment and visit bag supplies prior to home visits; ensure visit bag readiness and adherence to cold chain requirements where applicable. • Provide clear instructions to patients/families regarding pre-test requirements (e.g., fasting, medication timing, fluid intake) and post-collection care. • Collect blood and other specimens (e.g., urine, stool, swabs) as per the physician’s orders and organizational protocols, including pediatric and geriatric patients. • Label all specimens at the patient’s bedside immediately after collection, ensuring accurate patient identifiers, date, time, collector ID, and test requested. • Complete all laboratory request forms and electronic entries (e.g., WebEMR/LIS) accurately and in a timely manner. • Maintain strict adherence to infection prevention and control measures, including appropriate use of PPE, hand hygiene, and safe handling and disposal of sharps and biohazard materials. • Monitor patients during and after specimen collection for adverse reactions (e.g., syncope, bleeding, hematoma) and provide appropriate first-line management; escalate promptly to the nurse/physician as needed. • Ensure proper packaging, storage, and transport of specimens to the hospital laboratory within required timeframes and temperature conditions to preserve sample integrity. • Document all procedures performed in the patient’s medical record, including site of collection, complications (if any), patient tolerance, and any deviations or issues. • Document all procedures performed in the patient’s medical record, including site of collection, complications (if any), patient tolerance, and any deviations or issues. • Communicate promptly with the requesting physician, home care nurse, and laboratory staff regarding collection difficulties, sample rejection risks, or any concerns that may affect test results. • Participate in quality improvement activities related to the pre-analytical phase (e.g., monitoring sample rejection rates, mislabelled specimens, delayed transport) and implement corrective actions as directed. • Maintain stock levels of phlebotomy supplies in the home healthcare phlebotomy bag and report low stock or equipment issues to the responsible supervisor. • Ensure that all phlebotomy equipment (e.g., portable centrifuge, cool boxes, glucometers if applicable) is checked, cleaned, and maintained as per departmental guidelines. • Participate in orientation and ongoing training/competency assessments for phlebotomy practices in home healthcare, including new or updated procedures. • Contribute to patient and family education about the purpose of laboratory tests, how results are used in their care, and how to prepare for repeat or follow-up testing. • Maintain regular communication and collaboration with the home healthcare multidisciplinary team to support continuity of care. • Actively participate in unit Safety and Risk Management programs, including hazard identification, reporting of near-misses, and adherence to safety policies during home visits and driving. • Report incidents in the incident reporting system (e.g., needle-stick injuries, mislabelled or rejected samples, patient complaints) and participate in incident review/root cause analysis when required. Requirements: • Diploma or Bachelor’s Degree in Medical Laboratory Technology, Phlebotomy, Nursing or an equivalent recognized qualification. • Valid professional registration with the appropriate regulatory body (e.g., SCFHS) as per national requirements. • Valid Basic Life Support (BLS) certificate. • Valid Saudi driving license, if the role includes independent driving to patients’ homes.
