## Description

Duties & Responsibilities:
- Lead and facilitate detailed Root Cause Analysis for critical equipment failures and safety incidents.

- Use structured RCA methodologies (TapRooT, ICAM, 5‑Why, FTA) to identify technical, human, and systemic root causes.

- Develop recommendations and corrective actions with responsible departments.

- Maintain the RCA database, track implementation, and verify effectiveness of actions.

- Conduct post‑implementation reviews and monitor recurring failure indicators.

- Train internal teams on RCA techniques and investigative tools.

- Support Management of Change (MOC) reviews related to failure mitigation.

Competencies:
- Strong expertise in RCA tools (TapRooT, Apollo, etc.).

- Analytical thinking with ability to identify systemic causes.

- Excellent facilitation, interviewing, and communication skills.

- Technical understanding of equipment failure modes.

- Ability to evaluate evidence, timelines, logs, and technical documentation.

- Strong report writing and investigation documentation skills.

- Certified TapRooT® Investigator (Essential)

- Apollo Root Cause Methodology certification (optional)

- CRL

Minimum Requirements:
- Education:

Bachelor’s degree in Engineering (BE) or an equivalent qualification from a reputable university.
Preferably Mechanical, Electrical or Instrumentation

- Experience:

Min. 15 years relevant technical experience in engineering field in large Refinery or Petrochemical
Min. 5 years experience in RCA facilitation and defect elimination process is a must.
