## Description

Job Purpose / Objective :

Serves as a Pharmaceutical caregiver by efficiently recommending, activating, monitoring and evaluating patient–specific pharmacotherapy to ensure optimal outcomes. Ensures implementation of the pharmaceutical care plan by assuring the accurate and clinical resource for other pharmacy personal, medical and nursing staff, as well as other healthcare professionals.

Key Responsibilities / Accountabilities :

- Actively participates in patient care rounds with other members.

- Assist medical staff with the selection of drug therapy and patient monitoring.

- Review and interprets medication order for clarity, accuracy, rationally, compatibility, feasibility and suitability for the patient’s specific need.

- Assume responsibility for patient-specific pharmacotherapy plan to assure optimal outcomes.

- Determine workload priorities, verifies medication order, checks the final product for accuracy and assures timely delivery of the medication to the patient care areas.

- Act as drug information and education resource for patients, physician, pharmacy residents and students, colleagues, medication and nursing staff and other healthcare professionals.

- Document therapeutic recommendation in the patient’s medical record and pharmacy intervention forms/screens.

- Detect, analyze and report adverse drug reactions.

- Provide patient and family education as per needs.

- Participate in person-centered care initiatives undertaken by HMG.

- Enrich patient experience with compassion, respect and dignity.

- Perform other applicable task and duties assigned within the realm of his knowledge, skill and abilities

Requirements:
Education/ Professional Qualification:

- Masters’ Degree in Clinical Pharmacy or Pharma D

- The Manager of Pharmacy in cases may modify the experience requirement where an individual pharmacist exhibits exceptional knowledge and skills.

Experience:

- At least four (4) years of experience in Pharmacy.

Professional Licensing / Certification / Training:

- Certification: Must be licensable by Ministry of Health

- Licensing: Must hold a valid Saudi Council License
